Our main goal at Total Patient Care Home Health is to provide you with the best possible home care services you can receive while helping you achieve the highest possible level of independence. We do this through the employment of the most qualified nurses, therapists, social workers, and home health aides. In addition, we strongly believe in continuing education. Although our professionally licensed clinicians must meet continuing education hours to renew each license with their discipline-specific Board of Examiners, Total Patient Care Home Health also provides in-service training in a variety of areas throughout the year to all of its employees.
Home Health services are an excellent rehabilitation alternative for many patients. However, for services to be covered under Medicare, patients must meet the criteria for “homebound status”. Patients who might consider home care as a possible option include:
- Patients discharged from a hospital who are not fully recovered
- Patients with chronic conditions requiring on-going monitoring and treatment changes
- Patients who are not well enough to attend outpatient therapy
- Patients who have difficulty with ambulation and endure significant effort to get out into the community

TPC Bridge Program
TPC has developed a program offering in-depth long term planning. TPC employs a certified hospice/palliative nurse and a social worker with extensive palliative experience to assist patients and families understand and clarify long term goals, initiate and review advanced directives, provide education on future healthcare options, and assure a seamless transition to hospice when appropriate.
